Separate administrative information from clinical history

Begin with the minimum details needed to arrange a visit. Decide when consent, referral documents and clinical questionnaires should be collected inside the appropriate patient system. Avoid collecting sensitive histories through general marketing forms or public chat.

Design for families and accessible completion

A parent, guardian or support person may complete a form on someone else’s behalf. Review relationship verification, consent authority and accessible labels. Test incomplete submissions, corrections and mobile use before making a form part of your intake process.
